CARE COORDINATOR (Wraparound REACH Program)
We are currently looking to fill the position of a Wraparound Care Coordinator for the REACH Program. Successful candidate must have the ability to successfully engage youth and their families to participate in this voluntary program and coordinate the appropriate T19/Medicaid reimbursable mental health treatment services. Must have the ability to network and develop community based services and supports, provide effective crisis management, intervention and advocacy. Candidate must also have excellent verbal and written communication skills along with moderate computer related competencies. Knowledge of family systems, Milwaukee based community resources and the willingness to work with families in their homes and/or community settings are all required. The ability to work independently, experience working with families, and experience working with a diverse population is a must. Must have a BS or BA in Social Work or related field. Child & Family Therapist (TFC)
Currently looking for full time candidates. Requires an MSW or related degree and certification as an APSW or equivalent. Supervision towards 3,000 hours/MA # provided. Experience in case management, child and family therapy, and knowledge of the foster care system is required. Position offers flexible schedule. Crisis Intervention Worker for Supportive Permanent Housing Program
St. Aemilian-Lakeside is seeking a creative, innovative and flexible individual to work in our new Supportive Permanent Housing Program. The program provides housing and intensive case management services to 12 young adults ages 18-24 who experience mental illness and are homeless. This part time position supports program participants through bi-weekly contacts that are often in the evenings or on weekends. The crisis intervention worker collaborates closely with the program case manager, as well as other treatment providers and team members, provides crisis intervention, and completes all required documentation related to program participants. Associates degree required; Bachelor’s degree in social work or related field preferred. Additional qualifications include one year experience or more in social service, educational, or vocational programs working with teens or young adults who are high risk, and experience in networking and community involvement. 16 hours per week including weekends and evenings. Must have transportation and a valid driver’s license.
Independent Living Specialist
We are seeking a full-time Independent Living Specialist to work with foster youth ages 17-19 who live in our independent living apartment. Position requires Master’s level degree, MA Billable, APSW or equivalent required. Requires flexible schedule and some weekends/evenings. Ideal candidates have experience working in the child welfare system with adolescent foster youth and their families. Duties include: providing intensive independent living skills; case management; coordinating referral and placement; providing support to foster parents, collaborating with team members and crisis intervention.
Lead Care Coordinator (Wraparound Program)
We are seeking to fill the position of Lead Care Coordinator for the Wraparound program. Must have a BS or BA in Social Work or related field. MS/MSW preferred. Experience working with youth and their families to provide coordination of services, crisis management, intervention, and advocacy desired. Ideal candidate has knowledge of community resources, family systems, and conflict resolution. Candidate will have strong verbal and written communication skills. The ability to work independently, experience working with families, being able to handle crisis situations, and experience working with a diverse population is a must. Responsibilities to include consultation, clinical direction, and conflict resolution for care coordinators and Child & Family teams; initial and ongoing training for all care coordinators; monitoring of all out of home processes related to authorization; maintenance of quality assurance requirements for various Wraparound program components and assistance with supervisory and leadership responsibilities as requested. Positive Behavior Support Specialist (Day Treatment Program)
We are currently seeking a Positive Behavior Support Specialist for the remainder of the school year (through June 10, 2008) in the St. Aemilian-Lakeside Day Treatment Program to provide positive behavioral support to students. Responsibilities include active behavior management throughout the school and in classrooms, assistance in the development and implementation of behavior management plans, successful management of student behavior within the productive thinking room, expertise in use of approved Nonviolent Crisis Prevention Intervention techniques, create and maintain database of student behaviors including monthly reports of productive thinking room referrals and behavioral incidents. Position requires a high school diploma or GED. Bachelor’s degree in social work, special education or community mental health preferred. Previous experience working with students with severe emotional and behavioral disabilities.
Professional Treatment Foster Parent - Milwaukee Treatment Foster Care
Join the St. Aemilian-Lakeside foster care team in providing state of the art services to children & families. Be a part of an innovative foster care placement program in which candidates eligible to be licensed as Treatment Foster Parents are sought for full-time benefit-eligible employment. A competitive salary is provided and additional reimbursement for all caregiving costs at a specialized Treatment Foster Care rate.Applicant must have no additional employment. We are looking for qualified candidates to fill this position: Requires 5 years experience as a parent, with at least 1 year experience as a foster parent preferred. Home must meet all requirements for licensure under HFS chapters 56 and 38. Female foster youth with a history of chronic running away and other significant acting-out behaviors are to be placed until permanence is attained. Applicant must have own transportation, meet car insurance requirements and have a valid driver’s license. Program Coordinator
Requires an MSW or related degree, LCSW or equivalent & at least two years related experience preferred. Primary responsibilities include providing supervision of program therapists, ensuring services meet program, licensing & purchaser expectations and contributing as part of the TFC coordinator team in program growth & strategic initiative efforts. Youth Counselor
(Full-time & Part-time)
We are currently looking for full and part-time youth counselor. A Bachelor’s degree is required. Ideal candidate will have experience working with abused, neglected and emotionally disturbed children. 1st, 2nd and 3rd shifts are available to include weekends, evenings and holidays. Must have a valid driver’s license. A criminal background check will be conducted on all potential applicants. Salary is commensurate with experience.
Youth Transitioning to Adulthood Counselor
Seeking a creative, innovative and flexible individual to work as a counselor in our YTA (Youth Transitioning to Adulthood) Scholars program. This program provides long-term support and guidance to young adults who have been in foster care. The counselor position assists foster care alumni in negotiating the countless challenges related to learning to live independently. This position focuses on helping program participants to achieve employment, educational and vocational goals while developing responsible decision making skills, competencies in independent living and a productive attitude of independence. Qualifications include Bachelor’s degree in social work or related field, one year experience or more in social service, educational, or vocational programs working with teens or young adults who are high risk, and experience in networking and community involvement.
