Ongoing Case Manager

We are seeking human service professionals to join our team of Case Managers in Ongoing Case Management.  Overall responsibilities focus on the safety and well being of the clients we serve along with reunification and permanence.  In pursuit of our goals, Integrated Family Service case managers will perform home visits, attend meetings with family and services providers, perform data entry and reporting duties and participate in court proceedings.  Qualified candidates will have knowledge of child welfare services and functions plus an ability to demonstrate critical thinking skills; highly effective verbal and written communications skills are required.  Prior experience and knowledge of local community resources plus background working in diverse communities with youth and families are vital to success in this position.  A bachelor’s degree in social work or related field is required, candidates who hold a Master’s Degree and certification as a Social Worker preferred and are strongly encouraged to apply.  Successful candidates must have their own transportation, meet agency auto insurance requirements, and have a valid Wisconsin driver’s license with an acceptable driving record.  Spanish speaking/bilingual candidates strongly encouraged to apply; a pay differential offered after successful demonstration of language proficiency through bilingual testing.

Lead Ongoing Case Manager

We are seeking a Lead Case Manager for Ongoing Services.  A bachelor’s degree in social work or related field is required with a minimum of three years of supervised experience in family and child welfare; a master’s degree in social  work and certification by the state of Wisconsin as an Advanced Practice Social Worker is preferred.  Candidate will have knowledge of the laws and regulations pertaining to social services, children, youth and families and community resources.  Must be able to demonstrate critical-thinking skills and have highly effective verbal and written communications skills.

Overall responsibilities focus on the safety and well-being of the clients we serve, providing team leadership by role modeling and assisting with the development of team members, and providing assistance in the administration of program outcomes. In pursuit of our goals, the Lead Case Manager will perform usual case management activities on assigned cases, along with training, support/direction, and outcome tracking functions. Must have own transportation, meet agency auto insurance requirements, and have a valid Wisconsin driver’s license with an acceptable driving record.


Case Aide

We are seeking a Case Management Case Aide.  Associate’s degree in human services or related field required, bachelor’s preferred.  The ideal qualified candidate will have a minimum of five years of professional experience working with children and families in a social services setting, effective verbal and written communication skills, strong interpersonal relationship skills, the ability to work in a culturally diverse community and team environment, and will have strong computer skills.  Candidate must be able to work a flexible schedule including evenings. Candidate must be able to meet agency auto insurance requirements and have a valid Wisconsin driver’s license with an acceptable driving record.  Responsibilities will include supporting Case Managers as needed, providing transportation for client appointments, participating in home visits, assisting with scheduled supervised visitation and assisting with case files.  Spanish speaking/bilingual candidates strongly encouraged to apply.
